What is liposuction?

Liposuction removes localized fat deposits resistant to diet and exercise using thin cannulas to vacuum and reshape body contour. It is a body contouring procedure, not a weight-loss method, ideal for patients at or near their ideal weight.

Modern liposuction techniques are very different from older "fat removal" approaches. The goal is not just to reduce fat volume but to reveal natural body contours and create aesthetic definition. Which areas can be treated?

Liposuction techniques

Multiple liposuction techniques are available. The choice depends on the area, fat consistency, and desired result.

Tumescent liposuction (classical): A tumescent solution (saline + local anesthetic + epinephrine) is infused into the fat layer; the swollen tissue is then aspirated. Bleeding is minimal. The standard technique.

VASER (ultrasonic): Ultrasonic energy emulsifies fat cells, easing aspiration. Effective in dense fat areas (back, male abdomen, gynecomastia). Often improves subcutaneous contour smoothness.

How is the surgery performed?

Liposuction is performed under general anesthesia or sedation. Small (3-5 mm) incisions allow placement of cannulas to aspirate fat. Operative time depends on the number of areas treated, averaging 1-3 hours.

Single-area procedures (e.g., chin only) can be performed under local anesthesia/sedation as a day procedure. Multi-area and large-volume procedures use general anesthesia. Hospital stay is typically 1 night.

Is liposuction a weight-loss method?

No. Liposuction is a body contouring procedure, not a weight-loss method. It is intended for patients at or near ideal weight with localized resistant fat deposits.

Ideal candidate: stable weight, BMI under 30, with localized fat deposits resistant to diet and exercise. In obese patients (BMI over 30), liposuction is not the primary solution; weight loss should precede contour refinement. If a patient gains weight after liposuction, fat will accumulate in untreated areas (back, arms, face).

Preoperative preparation

Smoking should be stopped at least 4 weeks before surgery. Smoking impairs subcutaneous circulation and increases the risk of skin contour irregularities.

Blood-thinning medications (aspirin, NSAIDs, vitamin E, ginkgo, fish oil) are stopped 10 days before surgery. Standard preoperative tests are performed. Stable weight should be maintained for at least 1-2 months.

Compression garment is obtained preoperatively for postoperative use.

Recovery timeline

First week: Compression garment worn 24/7. Mild-to-moderate soreness, similar to muscle pain. Slight orange-tinged drainage from cannula sites in the first 24-48 hours is normal lipid leakage, not bleeding. Showering is generally possible from day 2.

Weeks 2-4: Compression garment continues 24/7. Swelling and induration (firmness) persist for months. Light walking can begin. Most patients return to office work in 7-10 days.

Weeks 4-6: Compression worn at night. Light exercise resumes. 50-60% of swelling has resolved.

Month 3: Most swelling has resolved; contour becomes clear. Full return to sports is safe.

Month 6: Final result. Skin completes adaptation to its new shape.

Lymphatic drainage massage: Professional lymphatic drainage 2-3 times per week for 4-6 weeks accelerates resolution of swelling and improves contour smoothness. Standard in modern liposuction protocols.

Risks and complications

Liposuction is a safe procedure but inappropriate technique or excessive volume removal can lead to serious complications.

Early-period risks: Bleeding (minimal with modern technique), infection (under 1%), seroma, asymmetry, contour irregularities (waviness, dimpling).

Skin contour issues: Excessive superficial fat removal can cause skin adhesions and irregularities. Modern technique preserves the subcutaneous layer to minimize this risk.

Serious (rare) risks: Fat embolism, fluid overload (in large-volume cases), infection spread. With accredited hospital settings and proper technique, these risks are well below 1%.

Skin laxity: Patients with reduced skin elasticity (older age, post-weight-loss, post-pregnancy) may experience skin laxity after liposuction. This may require an additional procedure (lift); preoperative assessment should anticipate this.
